The NRL assay measures intact PTH. The clinical utility of measuring PTH lies in evaluating calcium and phosphate balance and diagnosing parathyroid and bone disorders. In cases of hypercalcemia, PTH distinguishes primary hyperparathyroidism (elevated or inappropriately normal PTH with high calcium) from hypercalcemia of malignancy (suppressed PTH). In cases of hypocalcemia, PTH Identifies hypoparathyroidism (low PTH with low calcium) resulting from autoimmune destruction, genetic disorders, or prior neck surgery. In cases of CKD (chronic kideny disease), PTH Monitors secondary hyperparathyroidism caused by phosphate retention and low active vitamin D production.
