Methodology

High-pressure liquid chromatography (HPLC) tandem mass spectrometry

Remarks

Transfer separated serum or plasma to a plastic transport tube. Note: Label each tube with patient's name, collection time, and date. Submit specimens simultaneously on the same test request form. Test Overview

Measures 17-hydroxypregnenolone levels before and after ACTH stimulation to assess adrenal steroidogenesis. Helps diagnose congenital adrenal hyperplasia and enzyme deficiencies like 3-HSD deficiency

Specimen Type

Serum (preferred) or Plasma

Volume

1 mL (each specimen),

Specimen Container

Red-top tube, gel-barrier tube, lavender-top (EDTA) tube, or green-top (heparin) tube

Patient Preparation

No special preparation is required for this test. You may eat, drink, and take your medications as normal, unless instructed otherwise by your healthcare provider.

Collection

Draw blood for baseline 17-OH pregnenolone. Inject cosyntropin 250 ug IM or IV (If IV, dilute cosyntropin in 2-5 mL of sterile saline and inject over 2 minutes). Draw blood for stimulated 17-OH pregnenolone 60 minutes after.
